COVID-19 Policy:
PURPOSE & RESPONSIBILITIES
The purpose of this policy is to define the United Enterprises Inc. requirements for staff, employees & visitors regarding COVID-19. United Enterprises, Inc. staff, employees & visitors are all responsible for complying with this policy. Our main concern is the health and well-being of all United Enterprises, Inc. staff & employees, with the goal to mitigate the potential for transmission of an infectious virus at United Enterprises, Inc. This requires full cooperation among staff, employees & visitors. Only through this cooperative effort can we establish and maintain the safety and health of our staff & employees in the workplace. It is the responsibility of United Enterprises, Inc. management to follow all federal, state, and local regulations and guidelines.
POLICY
On scheduled days of work, United Enterprises, Inc. staff & employees are REQUIRED to sanitize their hands prior to entering United Enterprises, Inc. facility. Staff/Employees will only be required to undergo a temperature check if they are showing symptoms. If a staff or employee is running a fever of 100.0 or greater OR having COVID like symptoms, the staff or employee will be directed to go home and consult with their physician. The staff/employee is required to provide a physician’s note to management indicating the date staff/employee was seen by the
physician and the return-to-work date. If a staff or employee becomes ill while at work, they will be dismissed from work immediately and instructed to go home if the staff/employee has transportation. If a staff/employee must wait for transportation, the employee will be escorted to a location to self-isolate away from other employees, and the emergency person contacted to pick up the ill staff/employee. Once the ill employee is removed from the facility, that area will be sanitized entirely by a supervisor. That staff/employee will be instructed to consult with their physician; however, if a staff/employee has tested positive for COVID-19 and are WITHOUT symptoms (asymptomatic), but have tested POSITIVE for COVID-19, you must self-
isolate for (10) days from the date of your positive test. If you are, or become Symptomatic with a POSITIVE COVID-19 test, self-isolation MUST continue until the following criteria is met: 1) at least 10 days have passed since symptoms first started, 2) at least 24 hours have passed since the fever resolved (without the use of any fever reducing medication), 3) symptoms have improved (coughing, difficulty breathing, etc.) In the event that multiple staff/employee members become ill, the regularly scheduled group unit will be monitored as a ‘close contact’. Those staff/employee members who are vaccinated will need to watch for symptoms. Staff/Employees who are not vaccinated will need to watch for symptoms and wear a mask for 10 days. Day one will be the date of exposure. United Enterprises, Inc. staff & employees are REQUIRED to wear face coverings while operating or riding in a United Enterprises, Inc vehicle.

Prior to and after ALL lunch, breaks & restroom breaks, United Enterprises, Inc. staff & employees are REQUIRED to wash their hands. Hand washing signage is available in ALL restrooms as a reminder. United Enterprises, Inc’s supervisors also provide hand washing/sanitizing reminders as needed. United Enterprises, Inc. staff & employees are REQUIRED to practice physical/social distancing (6 feet apart) from each other while on the premises of United Enterprises, Inc. “Blue” X’s are displayed on the Production floor to provide a reminder of how far apart United Enterprises, Inc’s staff & employees must remain from one another to promote physical/social distancing. Where available, United Enterprises, Inc’s work areas are set up to practice physical/social distancing. In instances where United Enterprises, Inc. work areas cannot maintain physical/social distancing, face shields are added to provide an additional layer of protection. United Enterprises, Inc. will provide ALL PPE’s necessary to protect staff/employees from exposure.

 Face Coverings
 Face Shields
 Antifog Safety Glasses
 Gloves
 Hand Sanitizer Stations
 Hand Sanitizer on tables
 Hand Soap in Restrooms
 Disinfecting Wipes
 Disinfecting Cleaner

United Enterprises, Inc. provides contracted quarterly ventilation maintenance on ALL units.
Quarterly maintenance and filter changes occur to keep ALL units in good working & ventilation condition.
United Enterprises, Inc. supervisors are REQUIRED to sanitize ALL tables, chairs, and workstations each day after the employees finish their workday.
All United Enterprises, Inc staff/employees are encouraged to receive vaccinations as determined by the Centers for Disease Control and Prevention and local health authorities.
Staff/Employees will be required to complete and submit the attached “Employee Attestation of COVID-19 Vaccination Status” questionnaire to the Executive Director and/or Human Resources.
